Located in Victoria and Western Australia we are a team of individuals who make up a group of people with a passion! We are upskilled mental health workers. Our aim is to work collaboratively with referring partners and people they support. We are passionate about providing connection and meaningful support. Our team have the skillset which enables us to provide meaningful support work and participate within a Care Team to ensure continuity of care, as may be desired. We know connection is paramount to being understood, and being understood is the gateway to achieving your best life. We service the following catchments: VIC: Inner East Melbourne, Bayside, Outer East, South East WA: Perth, North, Inner, Western, Metro Our Mental Health Workers possess the following minimum requirements: - Studying in mental health or equivalent - First Aid & CPR Certificate - NDIS Worker Screening Check - Working with Children check - NDIS worker orientation module “Quality, Safety & You” What you can expect from us: - No long referral forms! An Initial discussion with you to review your needs - We will phone the participant to better understand their needs (as may be directed) - Face to face meeting with the participant at their home - Unique process to match our Mental Health Workers to the participant personality, plan and needs. - Regular participant progress updates We believe we can be the ones tho help participants to live their best life!
