Synergy was a pioneer in creating scalable healthcare solutions that integrated nutrition into clinical care, improving health outcomes and reducing healthcare costs. As part of its pioneering work, Synergy developed and validated some of the first food-as-medicine models, including the Kabixa benefit facilitation platform, Truck2Table clinical studies, and the GroceryVIP food prescription program. These innovations delivered over 100,000 healthy meals, demonstrated clinical and financial outcomes, and reshaped the way healthcare systems approach nutrition and wellness. Synergy was the first to deploy, orchestrate, and validate a functional healthcare benefit model that worked, yielding a 40% engagement rate and a 30% improvement in health outcomes. This work was done in collaboration with Humana beginning in 2018, and was among the first to address the importance of separating food insecurity from nutrition insecurity. Synergy also pioneered the deployment of SDOH screeners, well before they were widely adopted, ensuring that these tools integrated seamlessly into healthcare workflows.
