Take Control is a health care company that works with self-funded health plan sponsors, data aggregation companies, and healthcare consultants & service providers. We provide a full range of disease management and health coaching services to help improve your plan participants' health, reduce their overall health risks, and decrease healthcare costs. Typically, incentives are offered in the form of increased or reduced employee share of medical premiums, reduced or waived prescription costs, reimbursed gym memberships, and other methods. We contact high risk plan members to let them know about the incentives, and the benefits of improving their health. All plan members must meet established criteria to qualify for enrollment in our program. The professionals at Take Control teach our clients how to create a healthy lifestyle and "take control" of their health and wellness. We offer comprehensive and confidential education and support for each client's specific health care needs. Our unique telephonic delivery method allows clients to participate from work or home, and receive the individual attention needed to ensure success. Our health coaching includes intensive one-on-one monthly phone sessions with licensed dietitians, exercise and sports science trainers, and certified diabetes educators. Our Health Coaches are trained in nutrition, health education, corporate wellness, and behavioral health. Your plan members will be coached by our professionals based on your plan member's needs, as determined by their custom coaching plan. Just as every person is different - motivated differently, accomplish goals differently - our programs are not cookie-cutter, one size fits all; they are unique and dynamic - we constantly assess and either confirm they are on track, or re-direct and update their plan. This ensures better long-term success, and better clinical outcomes.
