About Tennessee Population Health Consortium
We are a consortium of major academic institutions, healthcare providers, and other stakeholders working to improve population health and health equity across Tennessee. Our mission is to encourage the adoption of evidence-based practices, transform primary and preventive care, and measurably improve health outcomes, quality of life, and health equity for the people of Tennessee. Our signature project is the TN Heart Health Network, committed to improving heart health for all Tennesseans through a statewide network of providers incorporating a patient-centered approach. Our other initiatives include the Tennessee Population Health Data Network (TN-POPnet) that tracks outcomes and quality of care for patients with or at risk of chronic conditions. The Diabetes Wellness and Prevention Coalition aims to improve diabetes care. And we train and certify healthcare workers in motivational interviewing and health coaching to expand the primary care workforce and improve population health.
