About The Middlewood Partnership
We are a general practice based in east Cheshire that has transformed our clinical care model to be fully integrated with the local community and social care partners. The formal merger of four separate practices into one organisation, the 165- strong Middlewood Partnership, allowed us to radically change the way we deliver care to our 34,000 patients in Bollington, Disley, and Poynton. Our integration with 'Team BDP' - our community and social care partners, including physical co-location - means we can manage patient care, particularly for those with the most complex needs, in the most effective, seamless way. This clinical model - with a single digital point of access to both the Middlewood and wider community teams - allows us to embrace every request, allocating it to the right professional to deal with quickly and responsively. We have redefined general practice for the 21st century: making it more responsive, integrated, engaged and resilient; establishing a stable, sustainable organisation and improving care for patients in the process.
