About The SCAN Foundation
The SCAN Foundation (TSF) envisions a society where all of us can age well with purpose. We pursue this vision by igniting bold and equitable changes in how older adults age in both home and community. Our grants and impact investments prioritize communities that have been historically marginalized with an emphasis on: older people of color, older adults with lower incomes, and older residents in rural communities. By integrating policy and programmatic grants with impact investing, we leverage a strategy that combines targeted initiatives with broader, mission-aligned investments. As a California-based foundation we also pay special attention to the needs of older adults in our state while leveraging learnings from California to support advancements in other states and at the federal level. Across our grantmaking, we start by listening to older adults and our priority populations to advance change that reflects what they say they need and design our programmatic and policy grants to address those concerns. We fund research, convenings, and community-based initiatives to advance equitable, timely, and accessible home and community care and supports for older adults. Our priority areas of focus include: • Advancing Long-term Services and Supports Reforms • Enabling Integration of Health and Social Supports in Medicare • Promoting Promising Models of Integrated Care for Dually Eligible Individuals • Championing Multisector Plans for Aging
