About Transition to Success® (TTS)

Transition to Success® (TTS) is transforming the nation’s understanding of poverty - from a stigmatized character flaw to a treatable, environmentally-based condition. TTS works by revolutionizing the way poverty is addressed. We have developed the first evidence-based standard of care to treat the condition of poverty, integrating uniform protocols and analytics to directly respond to the social determinants of health. TTS curriculums define specific treatment protocols with a 20-domain tool that assesses and measures social determinant exposures affecting heath and economic self-sufficiency. TTS trains direct care staff to teach clients to prioritize, identify and ensure access to all available services and supports across their community. CARE (Coordinating All Resources Effectively) management of existing resources integrated with peer mentoring, financial literacy and volunteerism empower clients (equipped with their CARE Plan) to achieve improved health, educational and economic success for themselves and their families. TTS Client Map of My Dreams Workbook & Guides are now available in 11 languages: Arabic, Burmese, Dari, English, French, Kinyarwanda, Pashto, Somali, Spanish, Swahili and Ukranian. Transition to Success has been shown to effectively improve social determinants of health in seven (7) independent evaluations with statistically significant results, All studies were completed without any additional funding for programs.
