Trenton Health Team has been serving Trenton for more than 15 years as a community healthcare collaborative dedicated to improving access to quality healthcare for city residents. The collaborative is an innovative partnership among St. Francis Medical Center, Capital Health, Henry J. Austin Health Center, and the City of Trenton Department of Health and Human Services. Over the years, our focus has expanded to meet the range of health concerns affecting our community. THT now operates with a broader understanding of what health and well-being — and health equity — mean for Trenton, as well as what activities and strategies will help address all the factors influencing a healthy city.
