United Community Health Center (UCHC) is a “one-stop shop” serving your medical and dental needs. UCHC is one of only thirteen Federally Qualified Health Centers (FQHCs) within the State of Iowa. United Community Health Center is privileged to serve our patients and community using a culturally diverse and experienced team of clinicians and other professional staff. We are also proud to have productive community, state, local and national partnerships that continue to enhance our ability to perform our work and thus increase our capacity to bring resources to you. We are truly committed to provide comprehensive and culturally appropriate primary and preventive health care services for your entire family. Care received at UCHC is not free – it is extremely affordable. UCHC receives federal grants to help eliminate disparities and to decrease barriers for those needing healthcare. These federal dollars supplement our budget and allows patients to see a doctor, dentist, or nurse practitioner on a sliding fee scale (starting at $20-$50 per visit), depending on your individual or family income. We encourage all people that are uninsured & underinsured, have high deductibles, and have Medicare, Medicaid, or private insurance seeking a Medical Home to consider us as your primary care provider.
