WorkWell Sustainable Team Wellnes and Prevention Program: The most effective health care is the kind of care that keeps people healthy, happy and safe from experiencing many of the avoidable chronic diseases that unfortunately afflict so many people today, such as obesity, osteoarthritis, chronic fatigue, heart disease and some forms of cancer. The WorkWell sustainable Team Wellness and Prevention program is all about helping you achieve your optimal wellness and to sustain good health by building new daily health habits. Along the way, you may also improve the quality of your life. The LiveWell Personal Wellness & Prevention Program was designed for individuals who want to make progress on their personal wellness indicators between annual physicals. Treatment first addresses any acute health concerns an individual may be experiencing and then advances to achieving preventative health goals. Patients are entitled to five visits with their doctor and are supported through the program over a twelve month period. Special discounted rates apply for families. Our use of Electronic Medical Files allows patients to book their own appointments and to interact with their doctor confidentially through a secure server.
