Welcome to our practice! We are proud to serve as your local Patient Centered Medical Home (PCMH) with locations at Happy Valley Medical Center and Collettsville Medical Center. We are committed to delivering quality healthcare to the whole person. We partner with our patients and their families to provide a medical home that is respectful, compassionate, accessible and comprehensive. Our FQHCs deliver high quality, culturally competent, comprehensive primary care, as well as supportive services such as health education, mental health/substance abuse services, oral care, translation, and transportation that promote access to care. We provide services according to the patients' ability to pay and charge for services on a sliding fee scale, dependent on an individual or family's income and size as it relates to the most recent Federal Poverty Guidelines.
