About Wisconsin Coalition For Serious Illness & Caregiving
Coalition History: Descriptive and analytical research was conducted through an MBA Social Entrepreneur Fellowship at UW-Stevens Point, with the goal to establish a Wisconsin Coalition for Serious Illness & Caregiving. In April 2023 a survey was sent to 350 WI stakeholders with results indicating overwhelming support and priority areas of caregiving, advance care planning, hospice, palliative care & community-based care and transitional services. Convened since May 2023, the coalition is now 77 members strong representing well over 80 organizations The Wisconsin Coalition for Serious Illness & Caregiving is a convener of health care and service providers who: "are committed to enhancing serious illness care in Wisconsin communities," work "to establish partnerships with individuals and organizations across Wisconsin to support informed decision making, address the impact of serious illness and access to care and services," and envision "engaging people in every day conversations about living with serious illness." WI Coalition members represent the following stakeholder groups: • Health systems • Hospitals • Health Plans • Hospice, Palliative Care and Home Health • Physicians, Nurses, Social workers, Clergy, Counselors • Advance Care Planning (ACP) Facilitators • Educators • Attorneys • Caregiving Advocacy Groups and Individuals • Managed Care Organizations • Government Program Leaders • Policymakers • Researchers • Funders • Quality Care Organizations • Community-Based Care Providers -SNF, Assisted Living, Memory Care • Human Service Agencies • Aging, Disability & Long-Term Care Providers • Independent Living Centers • Member Associations • VA Administration and Veteran Service Officers • Other Health & Service Providers State serious illness coalitions are supported by a national convener, the Coalition to Transform Advanced Care (C-TAC). Wisconsin is the 17th state to create a serious illness coalition.
