Mission Providing person centered conflict free quality case management. Vision: Assisting individuals to live their best lives. About Us: 4U Case Management provides conflict free case management for individuals who are eligible for the New Hampshire Developmental Disability Area Agency System. 4U Case Management has been named and developed with the focus on the individual receiving services. We are here to help you plan, design, advocate, implement and monitor the services that are best 4U. Conflict free case management is person-centered and promotes the rights of the individual. It separates the roles of the direct service providers from the case manager who is providing oversight and advocacy for those services. Currently, 4U Case Management is serving southern New Hampshire. 4U Case Management services have been developed from the personal experience of working, receiving, advocating, and developing NH developmental disability and mental health services over the past 25 years. 4U Case Management’s developer Brian Huckins has a history of being a family peer, direct support provider, home care provider, case manager, case management supervisor, NH START Coordination, Children’s Director at NAMI NH, and Senior Team Member at Greater Nashua Mental Health. Brian has been the Chair of the Governor’s Council to Study Autism and NH4Autism.
