Actualize Health is an accountable care organization (ACO) focused on expanding value-based care for underserved patients and the practices that care for them. For years, value-based arrangements have focused heavily on Medicare and Commercial patient populations. We believe underserved patients and their physicians deserve the same level of attention and support to improve outcomes.
