Physician-owned and established in 1995, we are New Jersey’s oldest and most innovative clinically integrated network and healthcare management company with extensive experience and expertise in accountable care, care coordination and population health… one patient at a time providing better health, better care at a lower cost. Our clients are commercial and managed Medicare health plans, self-insured employers and public sector entities, such as joint health funds, municipalities, school districts and the Partners In Care Medicare ACO. Our customized programs focus on improving employee and member health while lowering healthcare costs. Our Chief Medical Officer leads a clinical team of licensed nurses that leverages proprietary health IT technologies, tools and informatics to provide timely and targeted care for high-risk patients with chronic diseases and preventive care for lower-risk patients, which reduces more costly and avoidable condition deterioration, hospital admissions and emergency room visits. Our provider network is comprised of more than 650 primary care physicians, multi-specialty physicians and other healthcare professionals, located primarily in 14 of the 21 counties in Central and Southern New Jersey. We are a privately-held corporation owned by 47 individual shareholders all of whom are primary care and specialty physicians.
