About San Diego PACE
Providing Compassionate Care for Older Adults. San Diego PACE, a division of San Ysidro Health, Inc., is a specialized health plan that provides high quality, compassionate care to adults age 55 and older. Services are provided and coordinated by a team of geriatric specialized health care professionals who are bilingual and highly experienced in senior care. Our team of Geriatric Specialists is equipped to manage the care of our most frail seniors. Each participant is assessed by an Interdisciplinary Team (IDT) and a care plan is created for them. Our goal is to provide all around care for our participants in order for them to live well and thrive in their community. At San Diego PACE, we provide all care and services covered by Medicare and Medicaid (Medi-Cal in California) as well as additional services to help maintain the health and well-being of our participants with the goal of helping them live independently in their home and prevent the need for skilled nursing care. San Diego PACE offers easy access to care and closely monitors the health care needs of each participant which results in fewer health crises, ER visits, and hospitalizations, better management of medications, and improved quality of life.
